You have arrived on the Diabetes Local web page for the District of Columbia! Diabetes Local is an online resource for community members and providers who are interested in accessing diabetes and chronic disease management and prevention services. In conjunction with the DC Department of Health’s Diabetes Prevention and Control Program and local partners and providers, diabeteslocal.org provides a listing of community resources for individuals and health care providers to readily access.
The DC Diabetes Prevention and Control Program at the DC Department of Health aims to reduce the burden of diabetes in the District of Columbia through partnerships that promote infrastructure and environmental change, sustainable community programming, and quality improvements in the health care system. Visit the DPCP’s website at www.doh.dc.gov/diabetes to find out more information on the Diabetes Program and the status of Diabetes in the District.
Community Members:
To find resources near you enter your zip code in the top right corner. Here you will be able to find local services related to managing or preventing diabetes and other chronic conditions. You will also be able to find services for Nutrition & Weight Management, Physical Activity, Medication and Monitoring, Management Education, and other services for Chronic Conditions/Complications (i.e. Eye Care, Kidney, Smoking, and Heart Conditions).
Service Providers:
If you are a service provider you can add or update your resources to DiabetesLocal.org; doing so will allow your programs to be viewable to the DC community. Please note that all resources added are subject to review.
To add a resource you must first register as a DiabetesLocal user. Once you have registered you will be able to Create a Resource or Recommend a Resource to our library. If you have multiple locations or resources please use the DiabetesLocal Contact Us for assistance with multiple entries.
