ADA Recognized diabetes education program. Registered Nurses and Dietitians (Certified Diabetes Educators) provide comprehensive training to adults and children with type 1, type 2, gestational and pre-diabetes. This program includes individualized insulin training, pump initiation and continuous glucose monitoring services. This location also offers a Pump support group and a Type 1 friends (parents of children with diabetes) support group.

Johnston Memorial Diabetes Care Center is an American Diabetes Association recognized diabetes education center that incorporates physician care. The center is staffed with certified diabetes educators including nurses, a dietitian, and a physician. The center is dedicated to the comprehensive care of people with diabetes and their families through one-on-one and group education, insulin pump training, a diabetes support group, and physician care with Andrew S. Rhinehart, MD, FACP, CDE who has received American Diabetes Association and National Committee for Quality Assurance Provider Recognition for providing diabetes care.

Diabetes Self Management Education

Diabetes Self Management Education

Comprehensive diabetes education and support in group and individual sessions. Program is recognized by the American Diabetes Association for quality diabetes education.

Sessions led by experienced certified diabetes educators (nurses and registered dietitians). For more information call 540-689-1001.

Diabetes Prevention sessions available for those with prediabetes or risk factors for diabetes. Two session course covering basics of diabetes/prediabetes/risk factors, healthy eating, blood sugar monitoring (receive starter low cost glucose meter), physical activity (receive pedometer and walking tape). Call for dates/times and cost.

Diabetes Care--Separating Fact and Fiction sessions available for those diagnosed with diabetes. Portion of cost covered by Medicare and most insurance companies. Call for dates/times and cost. Scholarships available.

Diabetes Continuing Care monthy free group meetings covering various topics. Second Monday each month 7pm to 8:30pm at RMH Healthcare. No pre-registration required. Spanish sessions available.

RockD monthy free group meetings for children with type 1 diabetes and their families. Second Sunday each month 1:30pm at Rec Center. Contact Holly for more information.

ADA Recognized Program

Registered nursed and dietitian (CDEs) provide comprehensive training to adults with Type 1 and Type 2 diabetes as well as gestational diabetes and pre-diabetes. Includes an individual consultation with RN and RD, and 4 small group classes (meet weekly). Other services include Nurse Practitioner CDE visits to evaluate and manage insulin and insulin pumps for improved control. Most insurance accepted. Referrals from PCP and preregistration or appointments are required for class and services. Support group available.

Living Well WIth Chronic Conditions

No matter what your chronic condition, this evidence-based program helps persons with chronic problems. Low cost, offered two times per year at the Hospital.

St. Croix County offers Living Well With Chronic Conditions, a course assisted through the Greater Wisconsin Agency on Aging Resources. Stella and Chris are the team of instructors that operate class sessions twice per year for persons with chronic disease in the Baldwin and surrounding areas. This is the Stanford developed program on chronic disease management. Text books are $20, or can be borrowed during class sessions for a no fee course.
