Comprehensive diabetes education and support in group and individual sessions. Program is recognized by the American Diabetes Association for quality diabetes education.

Sessions led by experienced certified diabetes educators (nurses and registered dietitians). For more information call 540-689-1001.

Diabetes Prevention sessions available for those with prediabetes or risk factors for diabetes. Two session course covering basics of diabetes/prediabetes/risk factors, healthy eating, blood sugar monitoring (receive starter low cost glucose meter), physical activity (receive pedometer and walking tape). Call for dates/times and cost.

Diabetes Care--Separating Fact and Fiction sessions available for those diagnosed with diabetes. Portion of cost covered by Medicare and most insurance companies. Call for dates/times and cost. Scholarships available.

Diabetes Continuing Care monthy free group meetings covering various topics. Second Monday each month 7pm to 8:30pm at RMH Healthcare. No pre-registration required. Spanish sessions available.

RockD monthy free group meetings for children with type 1 diabetes and their families. Second Sunday each month 1:30pm at Rec Center. Contact Holly for more information.

Registered dietitian (CDE) provides medical nutrition therapy for children and adults with Type 1 and Type 2 diabetes, as well as for those with pre-diabetes, gestational diabetes, metabolic syndrome, and for weight management, cardiovascular risk reduction and many other chronic conditions. The program consists of a series of 4 classes. Topics include: Diabetes The Disease Process, Diabetes Medications and Monitoring Blood Glucose, Carboyhydrate Counting, Chronic Complications.

Education on diabetes management for beginners or control help for those with diabetes for years.

Baldwin Area Medical Center offers an American Diabetes Association Recognized Program for diabetes education. We offer group classes utilizing Conversation Map training or individual sessions with Registered Nurse, Certified Diabetes Educator and Registered Dietitian. We find small group education to be most fulfilling for the patient and provider and we like a little fun in our sessions. ThedaCare Diabetes Education Program

ThedaCare Diabetes Education Program

A diabetes self-management program for those newly diagnosed with diabetes or people who have had diabetes for years. Gain the information you need to manage your diabetes.

A Diabetes Self-Management Program accredited by the American Diabetes Association consisting of classes and individual sessions. Services also include medication and insulin pump management as well as continuous glucose monitoring systems (CGMS). Dietitians and nurses are available; most are Certified Diabetes Educators.

Gestational Diabetes education is also a service provided by a Certified Diabetes Educator at both Appleton Medical Center and ThedaClark.

All insurances accepted including medicare and medicaid.

Many locations available: ThedaClark in Neenah, Shawano, New London, Appleton Medical Center, Oshkosh and Fox Cities Community Clinic. Hours typically 8 a.m. to 5 p.m. Classes are available at varying times from 9 a.m. to 6 p.m. depending on location.

A physician referral is required. Referral can be faxed to (920) 831-5093 (Appleton Medical Center) or (920) 729-2921 (Theda Clark).
