Social service staff provide patients the support and assistance needed to resolve problems that interfere with their health. Staff provide patients with information and referrals for a wide range of services and needs, including assisting with applying for health insurance, getting prescriptions filled, where to obtain groceries, clothing, identification cards, how to connect to services for mental health, addictions counseling, education, employment, legal assistance, housing, shelter, and many other areas. Walk in appointments are accepted. To make an appointment with social service staff, please visit the health center. Health center hours of operation are Monday thru Friday 8:15 am to 4:45 pm.
